Diabetes Education Program - Kootenay Boundary

Provided by Interior Health

Provides education to increase diabetes awareness and prevention, and help individuals with diabetes manage their condition effectively including insulin adjustment.
Services are available for both adults and children affected by all types of diabetes including Type 1, Type 2, gestational (diabetes in pregnancy) and also pre-diabetes. Support partners, caregivers and family members are also encouraged to attend.

Services are offered in a team approach, which puts you and your family at the centre of your care. Nurses, dietitians and physicians with specific training and expertise in the area of diabetes management will work with you, your family/support partners, family physician and members of your broader healthcare team to support your ongoing and changing medical and self-management diabetes care needs.
Staffed by CDE Diabetes Educator RDs and RNs. Offer diabetes education, lifestyle coaching, and self management to individuals (and their families as indicated) who live with diabetes or Pre-diabetes in group or individual format:
  • Manage and assist with injectable therapies and oral medications.
  • Support with diabetes-related goals and action plans.
  • Educate and assess regarding vascular risk factors; includes foot assessments.
  • Specialty Diabetes populations: includes T1DM, GDM, T2DM-complex, Pediatric DM, Diabetes-related technology (insulin pumps, CGM).
  • Complex T2DM shared care clinics with Internist available in: Cranbrook, Creston, Fernie.
  • Pediatric endocrinologist shared-care clinics available in: Cranbrook and Creston.

    Patients are encouraged to see their Family Physician regularly for ongoing diabetes care and management.

    In person care is available in Trail, Nelson, Castlegar and Grand Forks. Services in Grand Forks and Castlegar may be limited. Remote and virtual care is available.
